Pharmacist License Renewals

This application is available for all licensees holding an active pharmacist license.  You may pay for your license renewal using a credit card [MasterCard or Visa] or ACH debit [checking account]. Upon completion of the online renewal process, you will receive a printable receipt. Your license will arrive by mail within 5 to 10 business days. To begin the renewal process, you must have your license number and the last 4 digits of your social security number. Please use the following link to begin your online renewal.

License renewals must be received in the Board office by close of business on February 28, 2010 [not post-marked].  All online renewals must be completed before 12:01 AM [EDT] March 1, 2010. All renewals received after February 28, 2010 will be assessed a delinquent fee of $70.00 pursuant to 201 KAR 2:050, Section 1(5).  Licensees accessing the online renewal process will automatically be charged a delinquent fee beginning March 1, 2010.

Those licensees holding an inactive pharmacist license are still required to submit their renewal application and $10.00 fee to the Board office.  Please use this link to print an inactive pharmacist renewal application.

Please remember, those pharmacists holding an inactive pharmacist license may not maintain an active license to practice in another state.
