Professional Licenses and Registrations



The Kentucky Board of Pharmacy issues the following types of professional licenses and registrations:

  • Pharmacist
  • Pharmacist Intern
  • Pharmacy Technician​

  • Please visit the Licensing Gateway to gather more information about licensing and the applications listed below. The Licensure Gateway is a single portal to manage licenses, permits, and registrations associated with the Kentucky Board of Pharmacy. It grants access to printable license/registration certificates, change of contact and employment information, online renewal applications and is used to submit all new license applications.

201 KAR 2:205 requires the pharmacist-in-charge to providing notification to the Board of Pharmacy within fourteen (14) calendar days of any change in the employment of the pharmacist-in-charge or staff pharmacists. The Kentucky Board of Pharmacy recommends all licensees/registrants update their employment as soon as employment change occurs.

​Address Changes

All licensees and registrants are required to notify the Board of any changes in address within 30 days of moving. Additionally, please keep your email address updated with the Board. Informational bulletins are now being sent by email. This mailing involves a "mass email program". Some email settings do not allow mass email messages. They will interpret these emails as spam. Please check your settings to allow emails from the Pharmacy Board.

Name Changes

All licensees and registrants are required to submit notification of name changes within 30 days. Please allow 3-5 business days for processing prior to reprinting the updated license/certificate from the website. All name changes must be supported with at least one of the following: a marriage certificate, a divorce decree, or an official court document authorizing the name change.​​
