Facility Licenses and Permits

The Kentucky Board of Pharmacy issues the following types of facility licenses and permits. Non-resident facility applicants must complete non-resident f​​​​​​acility applications. 

Facilities in Kentucky are inspected by the Kentucky Board of Pharmacy. New facility applications must be inspected and approved prior to issuing a license/permit. Facilities must also be inspected and approved prior to relocating an existing license/permit. Once a completed application is submitted, the applicant will be contacted by a pharmacy & drug inspector.

The Licensure Gateway Portal​ is a single portal to manage licenses, permits, and registrations associated with the Kentucky Board of Pharmacy. It grants access to online applications and printable licenses/permits. Any facility modifications, such as name, location, ownership, hours, or employees should be completed via Licensure Gateway. 



Closing Procedures

Facilities must follow the steps below for closure.
  • Follow the procedures in accordance with 201 KAR 2:106.
  • Submit a written notice of closure to the Kentucky Board of Pharmacy via Licensure Gateway, DEA and the Drug Enforcement Branch and Professional Practice Branch (DEPPB) of the Cabinet for Health and Family Services at least 15 days in advance of closure.
  • Verify with DEPPB of the Cabinet for Health and Family Services that KASPER data is current and complete (if applicable).
  • Upon closure, submit the actual date of cessation of operations and the final controlled substance inventory via Licensure Gateway.​

Theft or Loss Procedures

If your pharmacy has suffered a loss of controlled substances, please complete DEA Form 106.  This form needs to be submitted online to the DEA.  Please submit a copy to the Kentucky Board of Pharmacy. Additionally, please complete the appropriate form below and submit to the Kentucky Board of Pharmacy. Ownership Changes

Refer to 201 KAR 2:470 to determine what constitutes a change in ownership. A change in ownership does require an application, filed with the Board at least five days prior to the transaction occurring, and will result in a license/permit number change. Any change requiring written notification only should be submitted to the Board within thirty days of the change occurring.

The changes below require an application, $150 payment and will result in a license/permit number change:
  • Asset Sale
  • Sole Proprietorship change [new owner or the addition of a partner]
  • New incoming owner of a facility that is not a corporation or LLC
The changes below require notification only and will not result in a license/permit number change:
  • Addition of new owners/officers in a corporation or LLC
  • Sale of corporation
  • Sale of stock in a corporation
