Non-Resident Pharmacy Permit Information

Pursuant to KRS 315.0351​, a non-resident pharmacy permit is required to provide prescription medications to citizens of the Commonwealth. Pharmacy permits expire annually on June 30th. Online renewals will be available beginning in May. A delinquent fee of $150 is assessed if permit has lapsed. The Pharmacist in Charge must have an active Kentucky pharmacist license.

Requirements for a non-resident pharmacy permit

  1. Complete the Non-Resident Pharmacy Initial Application and required documentation via Licensure Gateway.
  2. A payment of $150, paid online or by check, made out to the "Kentucky State Treasurer".

Requirements for a non-resident special limited pharmacy permit - Clinical Practice

  1. ​Complete the Non-Resident Special Limited Pharmacy Permit - Clinical Practice Initial Application and required documentation via Licensure Gateway.
  2. A payment of $150, paid online or by check, made out to the "Kentucky State Treasurer".

Requirements for a non-resident special limited pharmacy permit - Medical Gas 

  1. Complete the Non-Resident Special Limited Pharmacy Permit - Medical Gas Initial Application and required documentation via Licensure Gateway.
  2. A payment of $150, paid online or by check, made out to the "Kentucky State Treasurer".

Requirements for a non-resident special limited pharmacy permit - Charitable Pharmacy

  1. Complete the Non-Resident Special Limited Pharmacy Permit - Charitable Pharmacy Initial Application and required documentation via Licensure Gateway.
  2. A payment of $150, paid online or by check, made out to the "Kentucky State Treasurer".

Required documents for a new non-resident pharmacy permit:

  1. Copy of resident state pharmacy permit
  2. Copy of last inspection report
  3. Copy of DEA registration (if applicable)
  4. KASPER verification certificate for PIC (if pharmacy has DEA registration)
  5. License verification from resident state or primary source verification from resident state
  6. Sample label
  7. End-of-Day report for 7 days
  8. Memorandum of Understanding and Agreement (MUA)
  9. Disciplinary history (if applicable)
  10. On-call policy if pharmacy is closed 2 days per week

  1. ​Copy of resident state pharmacy permit since ownership change
  2. Copy of inspection report since ownership change
  3. Copy of DEA registration (if applicable)
  4. License verification from resident state since ownership change or primary source verification from resident state
  5. Sample label
  6. Memorandum of Understanding and Agreement (MUA)
  7. Disciplinary history (if applicable)
  8. On-call policy if pharmacy is closed 2 days per week
