Requirements for Initial Application for Pharmacist Licensure:
*Certificate of Graduation - Form provided with the application.
*Certificate of Intern Hours - Form provided with the application.
*Applicants for initial licensure as a pharmacist shall submit to a query to the National Practitioner Data Bank of the United States Department of Health and Human Services pursuant to 201 KAR 2:020 Section 6(1). This query will be submitted by the Kentucky Board of Pharmacy. There is a fee of $25 for this query. Make checks payable to 'Kentucky State Treasurer'. Please note: Effective January 1, 2019 the FBI CJIS Division is implementing a fee revision on fingerprint-based Criminal History Record Information checks from the current fee of $12.00 per request to the new fee of $13.25 per request. Any requests received on or before December 27, 2018 with the current fee ($12.00) will be processed as normal. Requests received on or after December 28, 2018 with the current fee ($12.00) will be returned to sender. We will accept requests received with the revised fee ($13.25) anytime, however the processing will be delayed until after the fee change takes effect. The combined fee for a fingerprint-based state and FBI CHAI check will change the total cost per request from $32.00 to $33.25.
*Applicants for initial licensure as a pharmacist shall submit to a criminal background check by means of a finger-print check pursuant to 201 KAR 2:020 Section 6(2). Once the above requirements are met, the Board will submit your eligibility to test to NABP. You should receive an email from Pearson Vue to shedule your exam.Exam scores for both the NAPLEX and the MPJE for Kentucky are available at the National Association of Boards of Pharmacy website, www.nabp.net
