Reciprocal Information

Requirements for Application for Pharmacist Licensure through Reciprocity.

 

*License Transfer Application from NABP 

 

*Submission to the Board of the official license transfer application received from NABP and payment of $250. Checks made payable to 'Kentucky State Treasurer'.

 

*Applicant for reciprocity for licensure as a Kentucky Pharmacist shall submit to a query of the National Practitioner Data Bank of the United States Department of Health and Human Services pursuant to 201 KAR 2:030 Section 2(6).  This query will be submitted by the Kentucky Board of Pharmacy.  There is a fee of $25 for this query. It may be included with the application fee of $250 or paid separately. Checks made payable to 'Kentucky State Treasurer'.

 

*Applicant for reciprocity as a pharmacist shall submit to a criminal background check by means of a finger-print check by the FBI pursuant to 201 KAR 2:030 Section 6(7). Fingerprint cards will be mailed to the applicant upon receipt of the official license transfer application. Background Check Instructions Please note: Effective January 1, 2019 the FBI CJIS Division is implementing a fee revision on fingerprint-based Criminal History Record Information checks from the current fee of $12.00 per request to the new fee of $13.25 per request. Any requests received on or before December 27, 2018 with the current fee ($12.00) will be processed as normal. Requests received on or after December 28, 2018 with the current fee ($12.00) will be returned to sender. We will accept requests received with the revised fee ($13.25) anytime, however the processing will be delayed until after the fee change takes effect. The combined fee for a fingerprint-based state and FBI CHAI check will change the total cost per request from $32.00 to $33.25. 

 

*MPJE Registration

 

*A passing grade of the Kentucky MPJE is required for licensure.
