Requirements for Application for Pharmacist Licensure through Reciprocity effective December 9, 2020 

*MPJE Registration

*A passing grade for the MPJE is required for licensure.

*License Transfer Application from NABP 

 

*Submission to the Board of the official license transfer application received from NABP and payment of $250. Checks made payable to 'Kentucky State Treasurer'.

 

*Applicant for reciprocity for licensure as a Kentucky Pharmacist shall submit to a query of the National Practitioner Data Bank of the United States Department of Health and Human Services pursuant to 201 KAR 2:030 Section 2(6).  This query will be submitted by the Kentucky Board of Pharmacy.  There is a fee of $25 for this query. It may be included with the application fee of $250 or paid separately. Checks made payable to 'Kentucky State Treasurer'.

 

*Until November 20, 2020 - Applicant for reciprocity as a pharmacist shall submit to a criminal background check by means of a finger-print check by the FBI pursuant to 201 KAR 2:030 Section 6(7). Fingerprint cards will be mailed to the applicant upon receipt of the official license transfer application. Background Check Instructions. *Effective December 9, 2020 - Criminal Background Check via IDENTOGO
  • Must pre-enroll online at https:/uenroll.identogo.com/ for Livescan or Cardscan Processing for a $51.25 fee.
  • Livescan is recommended for In State applicants as there are less rejections, quicker processing and tracking.  
  • If your fingerprint submission is rejected, the Kentucky Board of Pharmacy will notify you. You will then follow the same process for resubmission of your fingerprints.
